Our office has partnered with RetireFlo – a secure and easy way to submit your prescriptions, medical providers, and preferences to our office! RetireFlo allows us to gather information we need to research plan options that will suit your needs and preferences.
1. If you are not interested in Medicare Advantage, please skip the provider (doctor) section.
2. When entering the name of your healthcare provider, please include the full name of the provider along with the address where you see the doctor
3. You will be prompted to complete a Scope of Appointment form prior to submitting your information. While it is optional, please note that we cannot communicate your results without this signed form.
4. On the Scope of Appointment form, place your initials next to the products you want the agent to discuss. We cannot discuss any products you do not initial.
